Job Description: The RN collaborates with the patient, family, and health care team in the formulation of an individualized plan of care and overall outcome goals based on collected data and patients age specific needs. The RN works collaboratively to coordinate the delivery of care and is responsible for managing outcomes. Must demonstrate the knowledge and skills, along with the understanding of principles of growth and development to provide appropriate care. Utilizes expertise in assuming the responsibilities for the nursing process for patients with common reoccuring problems on assigned unit. Implements the appropriate care path in order to meet the patient's physical, spiritual, and psycho-social needs for the patients assigned. RN acts as a clinical coach and coordinates care and staffing on the unit level.
